At Pursuit Allied Health, we’re here to help you stay safe, independent, and confident in your own home. Our team of Physiotherapists and Occupational Therapists work alongside you, and anyone already supporting you, to help you get back to doing the things that matter most.
What is the Restorative Care Pathway?
This is a short-term, structured program designed to help you regain strength, improve your movement, and reduce your risk of falls, all in the comfort of your own home.
The program runs for up to 16 weeks and is tailored to what you need. There’s no set minimum, it’s about what works for you.

- Giving you, and the people who support you, greater peace of mind
How does the program work?
We keep things straightforward and build the program around you. Here’s what to expect:
- Getting started: Before anything begins, we take time to understand your situation, your goals, any risks, and who else is involved in your care. We’ll book your first visit and map out your session schedule from the start, so there are no surprises.
- Your first visit: A Physiotherapist will come to your home. We’ll look at how you move around; walking, balance, getting up from chairs, managing stairs; and talk with you about what you’d like to achieve. Together, we’ll set practical goals that mean something to you, and you’ll leave with a home exercise program to get started straight away. We will also let you know if a referral for Occupational Therapy input is required.
- Your regular sessions: We focus on real, everyday movements; the kind that make a difference to how you get through your day. Your exercises are updated as you progress, and if you have a care worker, we’ll make sure they know how to help you keep moving between visits.
- Checking in along the way: Midway through the program, we’ll do a formal review to see how you’re tracking against your goals and make any adjustments needed.
- Wrapping up: At the end of the program, we’ll measure the progress you’ve made, put together an ongoing exercise plan for you to continue with, and make sure you and your care team know what’s next.
Who is this for?
This program is funded through Support at Home and is available to older Australians who want to maintain or improve their independence at home. Your Support at Home provider can refer you, and our team will take it from there.
